Case Management-Coordinator of Community Services
Job Type: Full Time
Work Schedule: 8:30am-5:00pm, Monday through Friday (flexible as needed)

Base Salary: $65,000 per year. Potential Bonus: Up to $5,000 annually, $1,250 quarterly.

Total Care Services, Inc. (TCS) is seeking dedicated and compassionate individuals to join our team as Case Managers in our Coordination of Community Services program. At TCS, we are committed to delivering high-quality healthcare programs and services to our clients and their families. As a Case Manager, you will play a crucial role in supporting individuals with intellectual and developmental disabilities and their families in Maryland.
Job Responsibilities
Work collaboratively with clients and their families to identify needs, goals, and preferences
Provide information about community resources and support services
Empower clients to make informed choices and plan for their future
Assist clients in accessing service delivery systems, resources, and supports
Monitor and evaluate services to ensure they meet clients' needs and preferences
Work Environment: This is a hybrid position - work from your home office with local community visits.
Requirements
Passionate about supporting our clients and their families
Strong advocacy for individuals with intellectual and developmental disabilities
Proven experience in Case Management
Ability to enter case notes and meet and exceed billable hours with a minimum of 7.5 hours a day into LTSS Maryland
Excellent written and verbal communication skills
Excellent organization and time management skills
Technologically savvy
Experienced Community Based Services Case Management
Minimum of 1 to 3 years of Clinical Case Management
Bachelor’s degree in human services field, such as psychology, social work, sociology, nursing, counseling, criminal justice, etc.
Must be a licensed driver with reliable transportation, valid insurance, and a safe driving record
Must be fully vaccinated and pass all pre-employment qualifications
