Your Specific Responsibilities The incumbent is part of a global public health organization and is expected to support the Florida Department of Health in Hernando County in its mission to protect, promote, and improve the health of all people in Florida through integrated state, county and community efforts in its vision to be the Healthiest State in the Nation. The role incorporates agency values of Innovation, Collaboration, Accountability, Responsiveness, and Excellence in all aspects of work.
This nursing position performs specialized nursing care for the TB, Primary Care, and Specialty Care Clinic and performs 'back-up' duties for Epidemiology and Linkage to Care. The nurse works as coordinator of the Primary Care and Specialty Care Clinic and providers, maintaining continuity of health care services within the clinic setting. Works closely with the Senior Community Health Nursing Director-SES and physicians to promote continuity of care. Responds to notices of Communicable Disease reports and documents in Merlin. Uses independent judgment to perform services and works with all populations.
Duties included but not limited to:
Provides nursing assessment and case management of clients enrolled in the TB, Primary Care, and Specialty Care Clinic. Takes and records medical history, vital signs, and issues prescribed medications under protocol. Documents in the chart using the problem-oriented S.O.A.P. method. Teaches healthy lifestyles, family planning, and risk reduction in the clinic and community setting.
Provides referral information to facilitate continuity of health care. Advises clients of test results and counsels on preventive measures according to unit procedures or protocols.
Develops individualized care plans. Reviews abnormal lab reports and consults with physicians regarding treatment plans.
Interviews clients for evaluation and assessment of patient condition, counseling and providing treatment under Registered Nurse Standard of Care Protocols and DOH Hernando Nurses Standing Orders.
Provides nursing assessments for Sexually Transmitted Diseases and TB, explaining disease, complications, and prevention; offers nondirective counseling.
Provides back‑up services for Linkage to Care and for Epidemiology, including on‑call, surveillance, response activities, and documentation in MERLIN.
Provides pre and post HIV counseling, including educational components.
Assists physicians/ARNPs with medical procedures per DOH Hernando procedures.
Maintains inventory logs for medications issued. Orders medications through ADAP and/or PAC waiver.
Coordinates and consults with TB specialists; provides TB education and yearly TB updates to staff.
Submits program reports as indicated or requested.
Ensures confidentiality, data integrity, and appropriate access to electronic records.
Maintains and updates professional knowledge through workshops, seminars, in‑service events, and professional organizations.
Performs other related duties as needed.
Required Knowledge, Skills, and Abilities
Knowledge of Merlin, Epi Com, basic biostatistics, basic epidemiology, and familiarity with Excel, PowerPoint, and Epi Info.
Knowledge of theories and concepts of preventive health care.
Knowledge of nursing principles, practices, and techniques in specialized areas.
Knowledge of human anatomy and physiology.
Knowledge of community resources and organizations.
Knowledge of principles of adult learning.
Skill in applying nursing theory in specialized areas.
Ability to teach professional nurses and other members of the community through outreach opportunities.
Ability to assess and prioritize service and personnel needs in a specialized area.
Ability to communicate effectively.
Ability to coordinate and evaluate a specialized or educational nursing program.
Qualifications Minimum
Must hold a current, active Florida Registered Nursing license or a multistate license from another Compact State pursuant to Florida Statute 464 and Florida Admin Code 210-8.22 or 8.27.
Preferred
Experience using computers to document medical care.
Experience in the family planning clinic.
Experience in the immunization clinic.
Experience working in a medical office or clinic setting.
