Professional nursing position for an individual with advanced skills, providing specialized nursing care with the primary emphasis on the treatment of individuals with end-stage organ disease that are designated to have a solid organ transplant. This role will also provide continuity and coordination of patient care, conduct patient and professional education initiatives, and collaborate with multidisciplinary teams, community resources, and agencies. Your Job Requirements

Bachelor of Science Nursing Current license to practice professional nursing in the state of Texas – REQUIRED Current American Heart Association Healthcare Provider Basic Life Support certification – REQUIRED Specialty certification (CCRD, CCTC) - Preferred Proficient with technical office equipment including computers, printers, fax, copiers, and telephones Minimum of 2 years of RN experience in transplant, renal, or end-stage organ disease preferred Your Job Responsibilities

Coordinates the pre-transplant evaluation process and collaborates with the multidisciplinary team to ensure timely completion Participates in the patient selection committee Assures patient is listed with testing and method of contact during the waiting period is clearly established Updates pre-transplant charts while waiting on the list at least annually Coordinates transplant admission process when organ offers are made for the potential recipient and backup recipients Communicates with third-party payers regarding transplant procedures and provides medical necessity documentation as required Coordinates and participates in patient/family education pre-transplant and maintains proper documentation Triages patient and/or donor calls based on transplant population needs Maintains data in checklists Data entry as required for database maintenance and patient management Maintains oversight required regulatory reporting including UNOS/HCFA/TIEDI, collaborative transplant study Coordinates and participates in patient/family education post-transplant and maintains proper documentation Monitors post-transplant patient clinical data and assures data is reviewed by a physician as required Communicates with patient and referring/primary physicians regarding changes in medications and other clinical management Conducts self as a professional, clinical expert in the area of practice, thus providing a role model for others Participates in educational programs sponsored by the Medical Center and those sponsored by outside agencies Consults with the staff of other units where the transplant patient is housed
