Position Summary
AdventHealth is seeking a dedicated and experienced Case Manager to work with patients and families to address the emotional, social, and financial impacts of illness. The role involves assessing readmissions, coordinating multidisciplinary care, and advocating for patient and family empowerment throughout hospitalization and discharge planning.
Key Responsibilities
Communicate with and educate patients and families regarding the emotional, social, and financial impacts of illness.
Mobilize family and community resources to meet identified needs and advocate for patient and family empowerment in health‑care decisions.
Assess readmitted patients for perceived reasons for readmission and identify barriers to progression of care.
Organize and facilitate patient and family care conferences with the multidisciplinary team.
Document discharge planning evaluations, ongoing assessments, and discharge plans per standard work.
Provide grief counseling, disease‑adjustment support, crisis intervention, and goals‑of‑care planning as appropriate.
Communicate with payors for authorization of post‑acute care and other services.
Leverage technology and best practices to ensure accurate discharge reconciliation and continuity of care.
Participate in multidisciplinary rounds to review changes in patient status and identify resources necessary at discharge.
Escalate care delays to leadership and perform other duties as assigned.
Qualifications
Master’s degree (required).
Relevant certifications: Accredited Case Manager (ACM), Certified Advanced Practice Social Worker (CAPSW), Certified Case Manager (CCM), Certified Independent Social Worker (CISW), Certified Social Worker (CSW), Clinical Social Worker License (LCSW), Licensed Baccalaureate Social Worker (LBSW), Licensed Master Social Worker (LMSW), Licensed Masters Social Worker‑Advanced Practice (LMSW‑AP), Licensed Social Worker (LSW), License Specialist Clinical Social Worker (LSCSW).
