in Topeka, KS. This role offers a 3,000 $ hiring bonus and covers weekend shifts (Friday‑Sunday, 8 AM‑8 PM).
Responsibilities
Assumes responsibility for patient visits or on‑call schedules, including assessing, planning, implementing, and evaluating nursing processes.
Provides hospice nursing care with systematic assessment focused on pain control and symptom management.
Initiates communication with attending physicians, hospice staff, and other agencies to coordinate care.
Maintains regular communication with supervisor to review assignments.
Maintains up‑to‑date documentation at point of service, ensuring accurate plans, actions, and goals, and reflects charges per agency policy and Medicare regulation.
Administers medication in accordance with federal and state laws and the Hospice policies.
Maintains current patient records, ensuring accurate interventions, goals, and outcomes.
Assesses and collaborates with Medical Director to determine initial and ongoing eligibility of hospice patients.
Completes an initial assessment of patient and family to determine home‑care needs.
Anticipates end‑stage disease process and related pain and symptom management.
Regularly re‑evaluates patient’s nursing needs.
Uses assessment data to determine care plan needs.
Initiates appropriate preventive and palliative nursing procedures.
Counsels the patient and family in meeting end‑of‑life needs.
Performs a comprehensive death note when appropriate.
Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition.
Communicates with other IDT members to coordinate the care plan.
Participates in on‑call duties as defined by the on‑call policy.
Ensures arrangements for equipment and other necessary items and services are in place.
Qualifications
State‑certified RN license (or LPN as noted).
Associate’s Degree in Nursing.
Experience in hospice or home health care (required).
Excellent documentation skills and ability to work independently.
Strong communication skills.
Valid driver’s license and reliable transportation.
Availability to travel locally within assigned territory.
