Pay range for this position is $37.05–$54.54/hour. Actual pay is based on years of licensure. This is an Hourly position.
OSF will invest in you day 1! Candidates with 5+ years of direct experience will receive a bonus of 24 hours of Vacation/Holiday time front‑loaded on day 1. This is in addition to our generous Paid Time Off plans!
$3.25 Weekend Differential
Overview The RN Case Manager – Home Health is responsible for identifying, assessing, planning, implementing, monitoring and evaluating clinical outcomes, service utilization and resource management for patients on their caseload. The Case Manager handles care across all ages, from infancy through geriatric, and collaborates with providers to coordinate interdisciplinary services that may include nursing, therapy, social work, home care aide, respiratory therapy, home medical equipment and home infusion services. The Case Manager is accountable for attaining the best possible patient outcomes, including focused oversight of patients at high risk for hospitalization, and provides evidence‑based, patient‑centered care at the patient’s place of residence using critical thinking and decision‑making skills. A thorough understanding of OASIS documentation and plan of care development, and their impact on 5‑Star Agency Performance and Value‑Based Purchasing performance, is required.
Qualifications Required Qualifications:
Education: Completion of a nursing program
Experience: RN in a healthcare environment for two years or more
Licensure/Certification: RN License in IL or MI, depending on state of work
Current American Heart Association HealthCare Provider BLS required prior to start date
Valid State Driver’s License with proof of active auto insurance
Excellent interpersonal and communication skills
Solid computer skills, including proficiency with Microsoft software
Strong analytical and problem‑solving skills, detail‑oriented
Provides own transportation and auto insurance; abides by all laws, rules and recommendations for safe driving while on duty; maintains a valid driver’s license
Preferred Qualifications:
Education: BSN
Experience: RN in Home Health, OASIS and Case Management experience
Licensure/Certification: OASIS Certification
Other Skills/Knowledge: Electronic Medical Record and Microsoft application basic proficiency
