Network Solutions IPA (NSIPA) is a leading New York–based Independent Practice Association with a diverse, multi-specialty provider network. The organization has grown to more than 600 physician members, serving over 100,000 patients across New York City, Nassau, Suffolk, and Westchester counties.
NSIPA provides a comprehensive suite of services, including credentialing and network operations, provider engagement and contracting; care coordination and member outreach; and provider & patient-centered programs. As part of New York State’s 1115 Medicaid Waiver, NSIPA plays a key role in identifying, screening, and addressing health-related social needs by connecting Medicaid members to community-based resources and care supports. Position Overview The Social Care Screener & Navigator is responsible for assessing members’ social needs, coordinating referrals to community‑based services, and ensuring all activities are completed in accordance with organizational standards and regulatory requirements. This role supports members by providing clear guidance, timely follow‑up, and professional communication throughout the navigation process.
Responsibilities
Conduct social needs screenings and assessments to identify member needs and determine appropriate service pathways.
Initiate and manage referrals through the designated Social Care Network (SCN) platforms, ensuring members are connected to the appropriate community resources.
Follow up on open cases to confirm service engagement, address barriers, and close cases within expected timeframes.
Escalate high‑risk situations, including positive depression screenings or crisis indicators, to the Team Lead or Director immediately.
Provide professional, culturally sensitive support to members through phone calls, messages, and ongoing communication.
Maintain accurate and timely documentation of all screenings, referrals, and member interactions in accordance with organizational protocols.
Use required technical tools and platforms effectively, reporting any system issues that may impact workflow.
Participate in required training, platform updates, and organizational briefings to stay current with program expectations and procedures.
Engage in consistent communication with the team, participate in scheduled meetings, and provide updates on progress and assigned tasks.
Identify and escalate operational challenges or barriers that may affect member care or performance expectations.
Ensure full compliance with HIPAA and confidentiality standards, safeguarding all protected health information.
Complete all required SCN trainings, certifications, including HIPAA and Mandated Reporter training.
Meet performance, attendance, and productivity expectations, contributing to team goals and overall program success.
Minimum Requirements & Qualifications
High school diploma or equivalent.
Ability to manage a caseload, follow structured workflows, and meet established timelines.
Ability to explain processes, next steps, and available resources in a way that is easy for members to understand.
Bilingual skills may be helpful depending on the member population but are not required unless specified by the program.
Strong attention to detail when entering information and documenting member interactions.
Comfortable using computers, web‑based platforms, and digital communication tools throughout the workday.
Excellent interpersonal and communication skills, with the ability to build trust‑based relationships.
Critical thinking and problem‑solving abilities.
Self‑motivated, organized, and able to manage competing priorities.
Proficiency in Microsoft Office Suite and other relevant software platforms.
High level of professionalism, integrity, and emotional intelligence.
