Responsibilities
Independently evaluates patients’ level of function and determines, develops, and implements individualized treatment goals and plans in alignment with patient/caregiver goals.
Delivers Occupational Therapy services that promote health and wellness, prevent injury or disability, improve and develop self-dependency and maintenance, productivity, leisure, perceptual motor function, rest, mental, cognitive physical, sensory-perceptual, psychosocial developmental, and emotional wellbeing in a variety of contexts and environments to maximize function in daily living, leisure/recreation, and productivity/work.
Delegates patient care responsibilities to Certified Occupational Therapy Assistant.
Performs thorough evaluation upon physician referral and evaluates care plan on an ongoing basis, revising as necessary in accordance with patient’s care needs.
Consults with other Health Care Team members as necessary.
Establishes measurable goals with the patient/family based on appropriate patient care protocols and critical paths.
Notifies physician of any changes in status or progress.
Delivers therapeutic techniques appropriate to patient’s needs and medical status, supporting everyday activities including fine motor skills, gross motor skills, and techniques to maximize function in the areas of daily living, leisure/recreation, and productivity/work.
Provides patient and caregiver education regarding treatment, precautions, and equipment use.
Completes all documentation including evaluation, care plan and goals, patient status and progress, visits, patient discharge summary, and patient care transition plan and summary.
Participates in patient transition planning and discharge planning, coordinating with community resources and other care providers such as Rehab Therapists, Nurses, Social Workers, and related resources.
Maintains clinical proficiency, continuing education units (CEUs), annual health stream, and all education requirements as required by the State of Connecticut licensure.
Qualifications
Education: Bachelor of Science Degree in Occupational Therapy required. Master’s Degree in Occupational Therapy or Doctor of Occupational Therapy preferred.
Experience: Previous experience as an Occupational Therapist preferred.
Licensure, Certification, Registration: Current State of CT Occupational Therapy license required.
Other: Perform other related duties as required.
