Working Hours: M-F; 8am-5pm; Some after hours and weekends possible.
Summary Responsible for the safe and timely transport of Life360 Program participants, their dependents and appropriate staff. Adheres to safe transfer techniques and infection control policies. Facilitates communication between Community Health workers and program leadership.
Other Information Education 21 years of age and high school diploma or equivalent required.
Languages English; Spanish is preferred.
Certifications Valid Driver's license.
Competencies
Knowledge about community resources appropriate to the needs of patients/families.
Effective time management practices (able to organize time, prioritize tasks efficiently and effectively).
De-escalation skills.
Problem-solving skills and managing change and conflict.
Knowledge of computer software.
Ability to develop trust and effective work relationships with staff members, individuals, families, and external and internal professionals.
Work Experience
One to three months related experience and/or training.
Preferred: driving for automobile delivery service, school bus, delivery truck, or equivalent.
Physical Requirements
May need to move around intermittently during the day, including sitting, standing, stooping, bending, and ambulating.
May need to remain still for extended periods, including sitting and standing.
Work evenings and weekends may be required.
May need to lift boxes or carry objects.
Possess dexterity to operate a PC and other office equipment.
Personal Protective Equipment Follow standard precautions using personal protective equipment as required.
This job will be authorized 80.00 hours bi-weekly.
