Healthcare Coordinator, LPN The Healthcare Coordinator (LPN)

partners with the Community Director to deliver exceptional care to residents. This role oversees care programs, leads and trains Resident Assistants, coordinates healthcare staff, and ensures compliance with state regulations, company standards, and HIPAA guidelines.

Key Responsibilities:

Assess residents’ needs and develop individualized service plans.

Maintain accurate Electronic Health Records (EHR) and monitor medication administration.

Notify healthcare providers and families of resident health changes.

Recruit, train, supervise, and evaluate healthcare staff.

Conduct staff in-services and ongoing training.

Manage inventory of nursing supplies and equipment.

Ensure proper use of personal protective equipment and safe handling of hazardous materials.

Provide 24/7 on-site coverage for emergencies and urgent resident needs.

Document incidents and follow up as required.

Maintain budget compliance for healthcare operations.

Qualifications:

Licensed Practical Nurse (LPN) in good standing with the state.

Current CPR certification.

Minimum 2 years’ experience in senior living or long-term care, preferably with dementia care.

Strong organizational, leadership, and interpersonal skills.

Ability to work flexible schedules, including weekends and occasional overnight travel.

Preferred Skills:

Experience leading and training direct care staff.

Knowledge of state regulations and long-term care best practices.

Ability to work collaboratively in a multidisciplinary team.
