Job Summary The Community Bridges, Inc. (CBI) Client Care Coordinator is a shelter or temporary lodging site‑based position whose primary role is to manage a housing caseload and coordinate care for individuals and families experiencing homelessness. The coordinator will provide care coordination and support services to help clients end their current episode of homelessness, often addressing complex substance use, mental health, housing, and medical needs. The coordinator ensures coordination of care with existing PCMH/PCP services or connects clients to new PCMH/PCP services and serves as the bridge for those residing in shelter or temporary lodging, coordinating and monitoring RBHA and Non‑RBHA community‑based services and facilitating delivery of these services.

Work with individuals and families with the highest acuities and greatest vulnerabilities.

Coordinate care through the CBI Continuum of Care or other community agencies identified by client choice on identified social determinants of health. These programs may include crisis, urgent care, residential treatment, medical care, medication‑assisted treatment, or outpatient behavioral health services. Once a client is matched to housing, assist with housing‑search software systems to help identify suitable units.

Complete data entry into established data systems to support program outputs and outcomes, assist higher‑level personnel and leadership with ongoing reports, and submit an end‑of‑shift report documenting care coordination for each individual.

Collaborate with a multidisciplinary team that includes Patient Care Techs, Registered Nurses, Counselors, and Clinical Leads. Coordinate care with community providers and stakeholders, and document required interactions per the Arizona Administrative Code (AAC), Arizona Department of Health Services/Department of Behavioral Health Services (ADHS/DBHS) Provider Manual, Commission on Accreditation of Rehabilitation Facilities (CARF), Community Bridges policies, and specific program contract requirements.

Skills & Requirements

High school diploma or GED required. Associate degree (or higher) in a field related to behavioral health preferred.

Minimum of six months of recovery from substance use and/or mental health disorders required.

Minimum of 12 months of paid related work experience providing adequate exposure to both behavioral and medical crisis situations required.

1‑3 years of full‑time health‑care related work experience providing adequate exposure to both behavioral and medical crisis situations preferred.
