Responsibilities Salt Lake Behavioral Health is seeking a Per Diem Program Therapist (Social Worker/(A)CMHC) for various units.

Schedule: Per Diem

Job Description: As part of the social services team, this position will complete biopsychosocial assessments, develop treatment plans, provide individual/group therapy and assist in discharge planning. This position will work collaboratively with the treatment team, families, community resources, and other healthcare providers.

Job Essentials

Prioritize daily tasks including new admissions and discharging patients along with seeing patients on given case load.

Participate daily in a multi-disciplinary treatment team meeting.

Conducts psychosocial assessments, ASAM’s and health disparities form for new admissions.

Educate, communicate and coordinate discharge plans with patient’s families and support systems.

Provide daily individual therapy along with psychoeducational groups.

Complete discharge planning including crisis safety plans, family meetings and discharge summaries.

Complete documentation in a timely manner according to the given standards of the hospital.

Compensation

Pay Range CSW/ACMHC (Exempt): $32.30-$42.80/hr

Pay Range LCSW/CMHC (Exempt): $36.30-$46.80/hr

Qualifications

Education: Master’s Degree from an accredited college or university, in Social Work or a clinical related mental health field required.

License: CSW, LCSW, ACMHC, CMHC, AMFT, LMFT with the state of Utah

Additional Requirements: CPR certification is required. You will need to successfully complete our current training for restraints and seclusions. This training will need to be done prior to assisting in any patient restraints or seclusions and within 90 days of employment.
