Base pay range $105,000.00/yr - $130,000.00/yr

Major Responsibilities

As the SNF-ist, you will act as a Skilled Nursing Facility’s leading provider of internal medicine services.

You will collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all of the facility’s residents.

You will work with the entire facility’s patient population—short and long-term residents—overseeing their day-to-day care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians.

Evaluation of new-admission visits.

Coordination of discharge visits.

Monthly stability visits for long term residents.

Evaluations for patients exhibiting changes in condition.

Provide advanced care planning.

Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.

Prescribe all necessary medications and treatments.

Perform comprehensive physical examinations of patients.

Develop and implement patient management policies and procedures.

Facilitate referrals to other healthcare professionals and medical facilities.

Communicate with attending physicians and/or specialists regarding patient care.

Qualifications

Advanced Practice Licensure (APN) with a Florida license.

Must have an active license and DEA number.

Previous experience in nursing or other medical fields.

Ability to build rapport with patients and their loved ones.

Ability to thrive in a fast-paced environment.

Excellent written and verbal communication skills.

Strong leadership experience with emphasis on teamwork in a healthcare setting.

Master’s degree required.
