Description Our full-time LVN will have a variety of duties that will help the company thrive and manage continuity of care. This candidate must maintain 40 productivity points weekly to qualify and must participate in the compensated on‑call program. This LVN will see all visits assigned within the scope of practice of their license. They will also participate in the compensated perception of new hires. Administer skilled nursing care for patients of all ages in their place of residence and coordinate care with the interdisciplinary team, patients, families, and referring agency. Assume the responsibility for, and participate in, the coordination of care.
Job Responsibilities
Be available Monday‑Friday to take patients.
Performs skilled nursing services under the supervision of a staff LVN.
Observes, records, and reports to the supervisor on the general physical and mental conditions of the patient.
Administers prescribed medications and treatments as permitted by State regulations.
Assists the physician and/or staff LVN in performing specialized procedures.
Assists the patient with activities of daily living and encourages appropriate self‑care.
Prepares clinical and/or progress notes and incorporates them into the clinical record at least weekly.
Reports to the patient's physician and other appropriate personnel pertaining changes in the patient's condition.
Performs complex wound care if in‑service is documented for specific procedure.
Performs routine venipuncture to obtain labs only if documentation of competency, by a staff LVN, is in the personnel record.
Receives and follows physician orders within realm of practice for an LVN and within the standards of home health practice but may not alter the plan of care.
Participates in in‑service program, attending at least two in‑service educational meetings per year.
Participates in nursing report.
Participates in staff conference regarding patient care.
Participates in QAPI plan and process.
Job Requirements
Active, state license as a Licensed Vocational Nurse, or a multi‑state license issued by a Nurse Licensure Compact (NLC).
2‑3 years of experience as a Licensed Vocational Nurse.
Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.
Proof of current CPR, TB, auto insurance, and COVID vaccine.
Reliable transportation.
