Responsibilities
Evaluate and treat residents in skilled nursing and post-acute settings. Develop and implement individualized treatment plans to help residents achieve their highest functional potential. Collaborate with an interdisciplinary team to ensure seamless, coordinated care. Document accurately and timely in accordance with facility and regulatory standards. Participate in care conferences and contribute to ongoing quality improvement efforts. Required Experience / Certifications / Licensure
