Aftercare Specialist The Aftercare Specialist reports to the Transition Aftercare Supervisor. This position consists of a lot of driving. The Aftercare Specialist is responsible for providing six months of follow up services to youth transitioning from a congregate care setting to the community. This includes at a minimum weekly phone calls to the youth, caregiver, and collaterals. It also includes at a minimum one in person visit a month as well as crisis intervention services.

Job Duties Include:

Participates in client staffings and completes necessary documentation and action plan items. Identification of stepdown resources and efforts must be done at admission and reported on during monthly staffings

Facilitates clients transition to next placement and oversees all required interaction, visits and documentation for billing as outlined in the DCFS Residential Discharge Protocol and in alignment with Family First requirements

Works with team to ensure youth receive swift, professional and high quality trauma informed treatment to most quickly move successfully to the next placement

Facilitates intensive aftercare services in next placement and provides support to the client and caregiver

Assists with discharge planning i.e. participating in staffings, making referrals, etc.

Completes a transition plan and review during every monthly staffing while the youth is in care and at every post discharge staffing

Completes a Clinical Staffing Report for every monthly staffing and post-discharge staffings

Provides a minimum of monthly in-person visits with the youth, family or caregiver and weekly phone contact with youth, caregiver, and collaterals

Provides on going clinical assessment of the youth’s stability in the post discharge setting

Conducts consultation between internal agency staff

Collaborates with external community providers

Provides clinical support to staff of the placement resources

Provides support to the youth, their family and/or caregiver

Maintains availability for consultation initiated by the youth, family, or caregiver

Provides transition sessions to youth and family

Implements and modifies the Behavior Support and Crisis Plan

Participates in Child and Family Team meetings

Transports youth and their families to mandated court hearings, school appointments, doctor appointments or other case related appointments. Attends court hearings, case reviews, staffing, and meetings as needed

Participates in On Call duties

Required Skills

1. Proficient computer skills in a Microsoft Windows environment and adaptive skills to expand software application skills as necessary

2. Highly organized, yet flexible, and can handle multiple priorities/projects well; ability to manage tight deadlines

3. Ability to function well in a team environment

4. Ability to demonstrate project management, organizational, analytical, and presentation skills

5. Ability to respond promptly to customer (youth, families, contract holder) needs; solicits customer feedback to improve services

6. Resourceful in researching issues and developing solutions with minimal supervision

7. Excellent interpersonal and effective communications skills (verbal, written, effective documentation, listening)

8. Demonstrated ability to build professional relationships

9. Demonstrated problem-solving techniques

10. Ability to coach, manage, and develop team members

11. Ability to exercise discretion and ensure confidentiality

Qualifications Education

Bachelor’s in Human Service or related field or, minimum of five years of supervised clinical experience

Experience

1-3 years of progressive responsibility related to social services

Other

Valid driver’s license and acceptable driving record with OHU required levels of coverage for auto insurance
