Position:
Travel Licensed Practical Nurse, LPN PRN.
Pay:
$20 / hr. - $35 / hr., depending on experience and responsibilities.
The Hospice Travel Licensed Practical Nurse (LPN) is responsible for providing quality patient care and performing technical skilled care in the patient’s home. This position assists home health aides in fulfilling their responsibilities and is a Regional Travel Nurse who travels within Central Tennessee.
Responsibilities
Provides technical care to patients, receives assignments and reports, and administers prescribed medication under the supervision of a Registered Nurse, in accordance with agency policies and local/state regulations.
Observes, records, and reports all observations of the patient’s mental and physical care in progress notes and submits notes daily to the Patient Care Manager.
Obtains a complete report on patients to be seen prior to visits and provides a complete report of care performed during visits to the Patient Care Manager.
Performs special procedures/treatments instructed and supervised by an RN, such as IVs, NG tubes, and drawing blood; documentation of these procedures is signed by both RN and LPN and filed in the personnel file.
Notifies the Patient Care Manager and/or physician immediately of any changes in a patient’s condition that warrant attention, documenting changes in the clinical record.
Participates as an IDT team member.
Assists home health aides in performing their duties and assists the physician or RN in performing specialized procedures.
Participates in on‑call rotations according to schedule.
Is willing to travel to other offices.
Qualifications
Must be licensed in the agency state of operation.
Must have at least one (1) year of nursing experience and complete knowledge of technical skills required for nursing procedures.
Must successfully complete on‑site evaluations and competencies within ninety (90) days of employment and bi‑annually thereafter.
Possess an understanding of hospice care concepts and the role as a member of an interdisciplinary care team.
