Responsibilities
The Social Services Director oversees the operation of the nursing center Social Services Department according to the National Association of Social Workers (NASW) Code of Ethics while maintaining compliance with federal, state, and local guidelines, regulations, policies, procedures, and standards of care for specialty practice.
Fosters a climate, policies, and routines that enable patients/residents to maximize their individuality, independence, and dignity, providing the highest practical level of physical, mental, and psychosocial well‑being and quality of life.
May directly supervise two or more Social Services staff.
Accountable for Social Services practice, measuring outcomes to interventions, and departmental productivity.
Assures that a psychosocial assessment is completed for each patient/resident, identifying social, emotional, and psychological needs and strengths.
Ensures patient/resident discharge goals are identified at admission and documented accordingly; assesses each patient/resident for discharge.
Educates staff regarding the role of Social Services and the psychosocial needs of patients/residents and their families/significant others, including problems of aging and disability.
Qualifications
Bachelor's degree in social work or human services required.
Must possess any certifications/licensures required by the state of employment to practice in long‑term care.
