Your Mission:
The Memory Support Program Assistant plays a vital role in supporting the Center for Health & Wellbeing's memory support initiatives by ensuring efficient program operations and a high-quality participant experience. This position provides administrative, scheduling, data tracking, and coordination support for memory-focused programs, groups, and outreach activities. Working closely with the Memory Care Navigator and program leadership, the assistant supports participant intake, program logistics, evaluation, and communication while fostering an organized, welcoming, and compassionate environment

Key Responsibilities: Participant Support & Program Intake
Manage participant scheduling for memory support programs, groups, and related services
Handle intake calls, provide program information, and guide individuals and families through enrollment
Respond to inquiries via phone, email, and in-person with professionalism, compassion, and exceptional customer service
Provide outreach and follow-up to program participants
Program Coordination, Data & Documentation
Maintain and update resource databases, program materials, informational guides, and CRM systems
Track participation, engagement, and outcomes data to support program evaluation and grant reporting
Assist with data collection, surveys, reports, and documentation related to memory support programs
Outreach, Events & Team Collaboration
Assist with planning, logistics, registration, and on-site support for memory support groups, events, and outreach activities
Coordinate materials and communication to support smooth program delivery
Provide coordination of care with medical providers and community resources to support participant needs
Your Qualifications:
High school diploma or equivalent required
One to two years of experience in administrative support, healthcare, social services, community programs, or memory care preferred
Experience working with older adults, caregivers, or memory support programs strongly preferred
Possess current or ability to obtain a Level 1 Fingerprint Clearance Card is required.
