Position Summary The Social Worker plays a vital role in supporting the emotional, social, and psychosocial well‑being of our residents. This position works closely with residents, families, and the interdisciplinary care team to ensure high‑quality care and smooth transitions throughout a resident’s stay.
Key Responsibilities
Provide social services support to residents and families
Complete psychosocial assessments and care plans
Assist residents with adjustment to long‑term care and life changes
Support discharge planning and care transitions
Advocate for resident rights, dignity, and individual preferences
Communicate effectively with families, staff, and community resources
Maintain accurate documentation in compliance with state and federal regulations
Participate in care conferences and interdisciplinary team meetings
Qualifications
Degree in Social Work or related field (required)
Experience in a skilled nursing or healthcare setting preferred
Strong communication and organizational skills
Compassionate, creative, and resident‑focused approach
Ability to work collaboratively in a team environment
