Job Location Territory: Jackson, MI and surrounding area

Salary $65,000 – $70,000 USD

Responsibilities

Provide social work services in accordance with the physician’s plan of treatment and client care manager referrals.

Assist team members in understanding social and emotional factors influencing client care, promote healthy lifestyles and psychosocial wellness.

Assess and reassess social and emotional factors to help clients and families cope with daily living challenges and follow medical recommendations.

Counsel clients and families regarding facilitation of the medical plan of treatment.

Recognize and utilize community and family resources for long‑term community living or disability adaptation.

Develop care plans with case manager, physician, client, and family to address personal, financial, and environmental barriers; set appropriate goals and participate in discharge planning.

Maintain accurate, timely client care progress records per policy.

Stay abreast of social work trends, participate in in‑service programs.

Coordinate the total care plan, maintain continuity, liaise with other health professionals, attend and initiate client care conferences as needed.

Participate in the agency’s quality improvement program.

Provide staff education when requested by the client care manager.

Show initiative to set and meet objectives and find efficient ways to perform tasks.

Complete work, care, and documentation accurately within agency time frames.

Show minimal need for supervision and demonstrate self‑direction.

Qualifications

Master’s degree from an accredited school of social work (Council on Social Work Education).

Licensed social worker in the state where practicing.

Minimum of one year experience in social work, preferably in home care agency, hospital, outpatient clinic, rehab center, or mental health program.

Knowledge and skills to care for adult and geriatric populations.

Understanding of growth and development across the lifespan.

Ability to assess client data and interpret age‑specific needs.

Computer skills for effective use of the electronic medical record.

Valid driver’s license.

Current auto insurance.

Reliable transportation.
