Job Overview
Position Title:
Field Advance Practice Provider – Ohio
Anticipated End Date:
2026-01-30
Location:
Field-based; travel to client sites in Akron, Ohio and surrounding areas; occasional office attendance for meetings or training. Eligible for mileage reimbursement.
Work Shift:
Monday – Friday, 8 am to 5 pm (EST) with rotating on‑call.
Responsibilities
Provide services in home settings to patients.
Develop and implement clinical plans of care for adult patients facing chronic and complex conditions (e.g., co‑morbidity, limited resources).
Gather history, perform physical exam and diagnostics, then develop and implement treatment plans based on patient goals of care.
Meet patient and family physical and psychosocial needs with support from the company’s inter‑disciplinary team.
Educate patients and families about medication usage, side effects, illness progression, diet and nutrition, medical adherence and crisis anticipation and prevention.
Maintain contact with other clinical team members, patients’ physicians and providers to coordinate optimal care and resources in a timely manner and in compliance with state regulations and company policies.
Maintain patient medical records and documentation consistent with state regulations and company standards.
Participate in continuing education as required by state.
Prescribe medication as permitted by state prescribing authority.
Minimum Requirements
Master’s degree in Nursing (MS) or Physician Assistant Studies.
Current, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of Ohio.
DEA registration or eligibility.
CPR certification.
For the Carelon Health business unit, satisfactory completion of a Tuberculosis test.
Preferred Skills, Capabilities and Experiences
At least 2 years of experience in home‑based services, complex care cases, and hospice and/or palliative care.
Experience in in‑home care, bedside hospital, oncology, emergency room (ER) or intensive care unit (ICU).
Experience working with Electronic Medical Records (EMR).
