Job DescriptionJob Description Reporting directly to the CEO with a dotted line to operations (COO), the Nurse Practitioner plays a critical role in delivering high-quality, patient-centered care in the field of podiatry and wound care management. This position is designed to provide comprehensive clinical services to patients in skilled nursing facilities, assisted living communities, and home-based settings. The Nurse Practitioner will assess, diagnose, and manage acute and chronic conditions related to foot and lower extremity health. As well as complex wounds, in collaboration with podiatrists and interdisciplinary care teams.
This mobile role supports improved patient outcomes, reduced hospitalizations, and enhanced access to specialty care for vulnerable populations. This position serves as a clinical ambassador- bringing expertise, compassion, and continuity of care directly to the patient’s home environment.

Essential Functions Conduct thorough patient assessments to evaluate wound conditions and overall health status. Develop and implement individualized care plans for patients with various types of wounds. Perform wound debridement, dressing changes, and other advanced wound care procedures. Collaborate with teams, including physicians and nursing staff, to coordinate comprehensive patient care. Educate patients and their families on wound care management, prevention strategies, and lifestyle modifications. Monitor patient progress and adjust treatment plans as necessary to promote healing. Maintain accurate documentation of patient assessments, treatment plans, and outcomes in compliance with healthcare regulations. Stay current with advancements in wound care practices and technologies.
Qualifications Valid Driver’s license, reliable transportation and driving to patients home and or clinical facilities Active NP license with 1-3 of previous experience in a clinical setting, mobile care is a plus DEA License preferred; Familiarity with podiatry care, pain management techniques and-related conditions is ideal. Basic Life Support (BLS) certification is required; additional certifications in advanced wound care are a plus. Wound care certification (CWS, CWCN, CWOCN preferred, as those are accredited programs; WCC at minimum, as it currently not an accredited program), or eligibility to obtain certification from an accredited program. Experience with EMR charting; Drchrono experience is a plus Proficiency in using electronic health record systems for documentation and patient management. Ability to lift patients safely and assist with mobility as needed.
Competencies Skilled in wound evaluation, staging, evident-based treatment planning Proficient in debridement techniques and infection control Familiarity with vascular assessment and lower extremity care Knowledge of injury prevention and management Ability to build trust with patients in skilled nursing, assisted living, and home settings Compassionate communication with patients experiencing chronic pain or mobility limitations Ability to provide education and coaching for patients and caregivers on wound care and prevention Understanding of HIPAA, and infection control protocols Proficient in EMR systems and mobile charting tools Demonstrates confidentiality, ethical standards, and sound clinical judgement Flexible with scheduling and travel across assigned service areas
Compensation Full Time:
$120,000-170,000 annual salary Part Time:
$55.00-$85.00 per hour
