THIS ROLE IS HYBRID REQUIRING FIELD VISITS THEREFORE MUST RESIDE IN ONE OF THE FOLLOWING COUNTIES FOR CONSIDERATION (MACOMB, WAYNE, BARRY, VAN BUREN, KALAMAZOO, CALHOUN, ST. JOSEPH, CASS, AND/OR BERRIEN COUNTIES)

Job Summary
The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual's benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes.
Requirements An active and unrestricted RN license for the state of Michigan 3 years Clinical practice experience, e.g., hospital setting, alternative care setting such as home health or ambulatory care 1 year of healthcare and/or managed care industry experience. xsgimln 1 year of case management experience Case Management Certification CCM preferred
