- Scope of Services : Full Primary & Family Care, Peds, BH, Case Management, Womens Health including Pre & Post-Natal, Peds, On-site Lab, Chronic Disease Care/Mgmt, Preventative, Lab, Health Education, Immunizations, Transportation, Enrollment, Referrals and Specialty Services
- Compensation : $260,000 - $300,000 Salary (DOE) + Sign-on Bonus.

- Schedule : Monday - Friday, 8:30am - 5:30pm. No weekends, No inpatient duties, limited phone-call rotation
