Nurse Case Manager II (RN) Compensation Pay rate up to
$35.87/hr
Nurse Case Manager II (RN) Highlights Fully remote, field & telephonic role Patient-facing case management position Full-time,
40 hours per week Schedule:
Monday–Friday, 8:00 AM – 5:00 PM EST Covers Counties: Cook, Lake, Will, Kane, Winnebago, Rock Island
Nurse Case Manager II (RN) Responsibilities Utilize a collaborative case management process including assessment, planning, facilitation, and advocacy Conduct comprehensive assessments of referred members’ needs, eligibility, and benefit plans Determine appropriate case resolution strategies using clinical tools and data review Apply and interpret applicable criteria, guidelines, standardized case management plans, and regulatory standards Coordinate care using available internal and external programs and services Utilize case management and quality management processes in compliance with regulatory and accreditation guidelines Communicate effectively with members, providers, and interdisciplinary care teams
Nurse Case Manager II (RN) Requirements Active, full, and unrestricted
Registered Nurse (RN) license in the state of Michigan 3+ years of clinical practice experience
(hospital, home health, ambulatory, or alternative care settings) Healthcare and/or managed care industry experience xsgimln Case Management experience preferred Strong computer skills, including navigating multiple systems and keyboarding Excellent verbal and written communication skills Ability to multitask, prioritize, and adapt in a fast-paced environment Verifiable High School Diploma or GED
Preferred Qualifications Case Management Certification (
CCM
) Prior field or telephonic case management experience
