Position Summary: The Diabetes Educator will play a vital role in supporting patients with diabetes while collaborating with Remote Patient Monitoring (RPM) program. This role focuses on interpreting CGM and glucometer data, educating patients on lifestyle and disease management, and working with physicians to optimize insulin therapy. The Diabetes Educator enhances patient outcomes, improves provider efficiency, and ensures compliance with RPM program requirements.

Key Responsibilities:

Patient Education & Support: Deliver individualized education on diabetes management (glucose monitoring, medication adherence, nutrition, exercise, and complication prevention). Reinforce compliance with RPM requirements, including the 16-day transmission rule. Provide ongoing patient support to strengthen self-management behaviors.

Clinical Collaboration: Review CGM and glucometer data trends within the RPD platform. Collaborate with physicians on insulin dose adjustments and insulin pump therapy changes. Communicate clinical insights to providers to enable timely, evidence-based care.

Documentation & Compliance: Document patient interactions, education, and interventions in the RPD system. Ensure documentation supports CPT billing codes for RPM and CGM monitoring (e.g., 99453, 99454, 99457, 95251). Maintain audit-ready records that reflect patient data and interventions.

Program Development & Quality Improvement: Provide feedback to leadership on patient engagement strategies and program enhancements. Support Care Coordinators with diabetes-related questions and interventions. Assist with developing educational materials on lifestyle and glucose management. Serve as a subject matter expert to Account Executives, helping communicate the value of our RPM and diabetes programs to clients.

Qualifications:

Required: Licensed healthcare professional (RN, RD, PharmD, NP, PA, or equivalent). Certified Diabetes Care and Education Specialist (CDCES) credential required. Minimum 2 years’ experience in diabetes care, insulin management, or chronic disease education. Strong knowledge of CGM technology, insulin therapy, and pump protocols. Excellent communication skills for both provider collaboration and patient engagement. Familiarity with RPM workflows and billing preferred, but not required.

Key Competencies Ability to analyze CGM/glucometer data and identify trends requiring provider intervention. Skilled in collaborating with physicians on insulin and pump therapy. Strong patient education and motivational interviewing skills. Detail-oriented and organized with documentation and compliance. Effective team member across RPD program.

Job Type: Full-time

Work Environment Hybrid/remote potential after initial training period. Close collaboration with Physicians, Care Coordinators, and Remoted Patient Device teams. Requires extended periods of computer and phone-based work.
