What You'll Do:
Deliver
skilled physical therapy services
in patients' homes
Assess
mobility, strength, balance, and functional limitations
Develop and implement
individualized treatment plans
Educate patients and caregivers on
home exercise programs and safe mobility strategies
Monitor progress, document outcomes, and adjust care plans as needed
Collaborate with an
interdisciplinary care team
to ensure excellent patient outcomes
What You'll Need:
Active
Florida Physical Therapist (PT) license
Degree from an accredited Physical Therapy program
Reliable transportation and valid driver's license
Current
CPR certification
Strong communication and time‑management skills
Home health experience (preferred)
