Job Description AccordCare Home Health is a licensed home care agency seeking experienced Registered Nurses (RNs) to complete Start of Care (SOC) admission visits throughout North Delray through Vero Beach.

Job Responsibilities
Complete Start of Care (SOC) admission visits
Provide prompt and accurate communication regarding client medical status to the Nurse Supervisor to ensure effective case management
Travel throughout the Treasure Coast service area as needed
Demonstrate empathy, professionalism, and confidence when interacting with clients and families
Maintain thorough, accurate, and timely documentation
Anticipate potential issues and appropriately resolve or elevate concerns
Prioritize tasks and demonstrate a sense of urgency
Required Experience & Education
Active Registered Nurse (RN) license (Florida)
Current Level 2 clearance in the AHCA Clearinghouse
TB test within the last 12 months or Chest X-ray within the last 5 years
Prior home care experience required
Excellent communication, collaboration, and customer service skills
Ability to use an electronic health record (EHR) for documentation and care coordination
