Pain Management Nurse Practitioner – Outpatient Specialty Practice (North Country Healthcare / AVH)
Job Type: Full-time (0.8–1.0 FTE)
Schedule: 4–5 days per week, daytime clinic hours
Location: Berlin, NH (primary), Colebrook, NH (weekly outreach), plus potential North Conway outreach
Role: Advanced Practice Provider (APRN – not CRNA)
Compensation: $155,000 Competitive Base Salary, Good Citizenship Bonus opportunity, and Productivity Incentives
Job Description Androscoggin Valley Hospital (AVH) , a proud member of North Country Healthcare, is seeking a
compassionate and experienced Pain Management Nurse Practitioner
to join our growing multidisciplinary Pain Management team. This role supports expanding community need and offers the opportunity to make a meaningful difference in the lives of adults experiencing chronic and complex pain conditions.
Working within a collaborative specialty practice, you will provide
comprehensive, whole-person pain care , including medication management, counseling and education, outpatient minor procedures, and referral coordination with interventional pain colleagues. This is an outstanding opportunity for a mission-driven NP who values patient relationships, interdisciplinary teamwork, and high-quality evidence-based pain care in a supportive rural healthcare setting.
Key Responsibilities
Provide outpatient pain management services to adult patients, including assessment, diagnosis, treatment planning, and longitudinal care
Manage controlled and non-controlled medications responsibly, including PDMP review and urine testing when appropriate
Educate and counsel patients and families on pain management strategies and expectations
Perform minor office-based procedures such as:
Trigger point injections
Joint injections
Occipital nerve blocks
Botox for headaches and selected pain conditions
Cold ablation (Iovera)
Refer eligible patients for interventional procedures performed by CRNA colleagues (e.g., epidural steroid injections, ablation, facet or SI injections)
Provide certification for medical cannabis when appropriate
Participate in occasional inpatient/ED consults (rotation-based)
Maintain accurate documentation in Meditech Expanse EMR
Collaborate closely with CRNAs, specialty leadership, and primary care teams
Deliver compassionate, patient-centered care aligned with AVH values
Schedule & Practice Setting
0.8–1.0 FTE (4–5 days per week)
Typical clinic schedule: 8 am – 5 pm (lunch 12 – 1)
Locations:
AVH (Berlin): 3+ days weekly
Upper Connecticut Valley Hospital (Colebrook): 1 day weekly
North Conway outreach: expanding
Shared home-based call rotation (phone only; no onsite requirement; additional compensation)
Typical patient volume 12–20 per day
Procedure training and mentorship provided as needed
Benefits

$15,000 Commencement bonus
Additional compensation for shared call availability
Good Citizenship Bonus opportunity up to $10,000 annually

Qualifications
Board Certified Licensed Nurse Practitioner (APRN) — not a CRNA role
Active or eligible New Hampshire APRN license
Minimum 2+ years’ experience in pain management strongly preferred
Experience performing minor procedures preferred

Strong communication & teamwork skills
