Primary Function The Pharmacy Team Scheduler and Care Coordinator support clinical pharmacists by managing appointment scheduling, patient outreach, and care coordination across the Care New England Health System. This role serves as a central liaison between patients, providers, and health plans to ensure timely access to pharmacy services, accurate referral processing, and seamless communication. The coordinator will optimize scheduling workflows, manage re-bookings, and facilitate authorizations and referrals for medication therapy management, chronic disease support, and Centers for Medicare and Medicaid Services / Healthcare Effectiveness Data and Information Set (CMS/HEDIS) related initiatives.
Job Qualifications and Specifications High School or GED Required; Associate degree Preferred; Minimum 3 to 5 Years of experience in a medical or pharmacy setting with scheduling and customer service responsibilities. Medical Terminology certificate (or knowledge of)
