Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. Position Summary The Coordinator will work with partners to implement HIV prevention activities in Central Brooklyn. This will include the implementation of evidence based intervention workshops, outreach, HIV testing, and one on one risk reduction counseling.
Job Responsibilities
Conduct evidence based sexual health workshops in Central Brooklyn high schools and community based organizations.
Submit all required documentation
Participate in weekly staff meetings at Project Reach Youth
Participate in FHC community events to connect youth STI and HIV testing
Provide rapid HIV counseling and complete related documentation
Provide sexual health education to youth at Wingate SBHC
Promote FHC Flatbush Health Center services
Connect workshop participants to FHC School Based Health Center, FHC Flatbush Health Center, or other social services
Meet individually with workshop participants identified as "high risk" to provide risk reduction counseling using motivational interviewing
Administer substance abuse and sexual health risk assessment surveys to all workshop participants
Additional Position Specific Responsibilities Travel between sites may be needed.
Minimum Qualifications
To qualify you must have a Bachelor's Degree
Prior work experience with community health outreach. Health related background.
Preferred Qualifications
At least three years experience working with adolescents and young adults.
Good interpersonal skills.
Qualified candidates must be able to effectively communicate with all levels of the organization.

Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.
