for the Colorado Springs area. Sign‑on bonus $3,500.
LOCATION:
Colorado Springs, CO
STATUS:
Full‑time
SCHEDULE(S):
Saturday‑Wednesday 10:00 a.m.–7:00 p.m. or Monday‑Friday 11:00 a.m.–8:00 p.m.
PAY RANGE:
$77,376 – $90,916 annually
SUPPLEMENTAL PAY:
Paid mileage, shift differentials, stipends (based on position, schedule, and availability)
MILEAGE & EXPENSE REIMBURSEMENT:
$0.625 per mile – one of the highest in the industry!

Rewarding Work You Will Do
Provides assessment, supportive care and education to prospective patients and families regarding hospice services; determines hospice appropriateness; assists in admitting patients to the program.
Acts as a liaison with assigned hospitals, facilities, and physicians.
Assesses and counsels patients and families during initial interviews, gathers information, reviews charts, and records data; provides education on hospice philosophy.
Efficiently and accurately completes all admission paperwork and initiates the clinical record through the EMR process.
Assesses and places orders for immediate medication needs; oversees placement of durable medical equipment, supplies, and oxygen.
Consults with staff of hospitals, SNFs, or ALFs to determine discharge orders and facilitate transition to home, SNF, or other residence.
Interfaces with referring physicians, nurses, ancillary providers, and discharge planners to facilitate a well‑coordinated transition into hospice.
Completes the full Comprehensive Assessment at admission and inventory of current medications.
Communicates pertinent patient/family information to the interdisciplinary team and follows through to facilitate a smooth transition.
What We Are Going To Love About You
Minimum Education: Graduate of an accredited school of nursing; BSN preferred.
Minimum Experience: One to two years in hospice palliative and/or home health care, medical/surgical, public health, oncology, acute care or home health.
Required License: Current Colorado RN licensure, CPR certification, and valid Colorado State driver’s license with auto insurance.
Physical Requirements
Ability to lift a minimum of 35 lbs.
