Registered Nurse Home Health Case Manager – Norridge, Harwood Heights and Surrounding
Advocate Health Care is looking for an experienced Registered Nurse to serve as a Home Health Case Manager in the Norridge / Harwood Heights area (ZIP codes 60634, 60641, 60656). This is a direct‑patient‑care, full‑time position with day rotations, one summer holiday, and one winter holiday. Eligible for up to a $10,000 sign‑on bonus.
Base pay range:
$37.50 /hr – $56.25 /hr
Responsibilities
Develop and maintain therapeutic nursing‑patient relationships throughout the continuum of care.
Demonstrate adaptive communication, provide individualized care to promote physical and psychosocial comfort, and involve patients and families in care planning.
Apply a holistic, individualized plan of care and stay current with evidence‑based practice.
Mobilize appropriate resources, adapt practices, and participate in quality/process improvement initiatives to reduce risk and improve safety.
Serve as a patient and staff educator; assess learning readiness, modify approaches, and provide opportunities for staff skill development.
Use the nursing process as a problem‑solving model; evaluate clinical changes, implement plan modifications, prioritize care, and coordinate interdisciplinary referrals and discharge planning.
Delegate to unlicensed assistive personnel, ensure confidentiality, and address disruptive behaviors to support care delivery.
Lead and collaborate with the patient‑care team; communicate effectively, provide feedback, resolve conflicts, and pursue professional development.
Maintain up‑to‑date certifications or a Bachelor’s degree, with a minimum of 10 contact hours per year.
Licensure and Qualifications
Registered Nurse (RN) license, current in Illinois, with a valid driver’s license.
Graduate from an accredited nursing school.
At least one year of medical‑surgical experience.
Proficiency in the requirements of the RN role, ongoing continuing education (10 credits per year) and current CPR certification.
Strong communication, documentation, and computer skills; good time‑management and organizational skills.
Familiarity with patient‑care equipment.
Physical Requirements & Working Conditions
Work weekends, holidays, and overnight shifts as needed.
Take “on call” assignments and travel within the advocate service area in all weather.
Lift objects and patients: up to 100 lbs occasionally, 50 lbs frequently, and 20 lbs constantly; assist with 200 lb transfers; use patient‑handling equipment when necessary.
Provide patient care in potentially hazardous or adverse conditions, including exposure to infectious specimens.
Complete multiple tasks quickly and respond promptly to patient needs.
