LTSS Service Coordinator-Clinician Company

This is primarily a field based position. Candidates must reside in one of the following counties: Collier, Desoto, Glades, Hendry, Lee, Pasco, Pinellas, Sarasota. The role involves traveling to client sites or designated locations, with occasional office attendance for meetings or training. Schedule
Monday‑Friday 8am‑5pm EST Responsibilities
Assist the responsible RN in identifying members for high‑risk complications. Obtain clinical data as directed by the responsible RN. Assist the responsible RN in identifying members that would benefit from an alternative level of care or other waiver programs. Provide all information collected to the responsible RN, who verifies and interprets the information, conducts additional assessments as necessary, and develops, monitors, evaluates, and revises the member’s care plan to meet the member’s needs. Participate in coordinating care for members with chronic illnesses, co‑morbidities, and/or disabilities as directed by the responsible RN and in conjunction with the RN, member, and the healthcare team, to ensure cost‑effective and efficient utilization of health benefits. Make decision‑making decisions based on the current needs of the member, applying understanding of disease processes, terminology, and clinical guidelines without requiring nursing judgment. Minimum Requirements
Requires an LPN/LVN, LSW, LCSW, LMSW, or license other than RN in accordance with applicable state law and a Nursing Diploma or AS in Nursing or a related field. Minimum of 2 years of experience working with individuals with chronic illnesses, co‑morbidities, and/or disabilities in a Service Coordinator or similar role; or an equivalent combination of education and experience. Current, unrestricted LPN/LVN, LSW, LCSW, LMSW, or license other than RN (as allowed by state law) in applicable state(s) required. Preferred Skills & Capabilities
Bilingual in Spanish highly preferred. Long‑term care experience highly preferred. MA/MS in Health/Nursing preferred. May require state‑specified certification based on state law and/or contract. Travel to worksite and other locations as necessary is preferred. Job Level
