Qualifications Education:
Minimum bachelor's degree in Social Work or related human services degree; licensure/limited licensure preferred. Must be eligible as QIDP or QMHP.
QIDP qualifications:
Specialized training with IDD or one year experience with IDD; profession includes psychologist, physician, educator, social worker, PT, OT, SLP, audiologist, behavior analyst, RN, RD, recreation specialist, licensed counselor, or human services professional.
QMHP qualifications:
Specialized training with mental health or one year experience with mental illness; include same professional categories plus LCSW, LMFT, physician's assistant.
Experience:
One year working with IDD or SPMI population; proficient with Office 365.
Additional skills:
Ability to interact with physicians and other healthcare providers; knowledge of physical and mental health and behavioral medications.
Summary of Responsibilities The Supports Coordinator provides service coordination and case management for persons with severe and persistent mental illness or intellectual disability, with or without co-occurring substance use disorder, following a Person-Centered Plan. Responsibilities include liaison work with clients and providers, advocating for inclusion, ensuring quality service delivery, and maintaining accurate clinical documentation.
Essential Job Functions
Develop, review, deliver and modify Person-Centered Plan of Service collaboratively.
Act as liaison between service providers, primary care physicians, and community services.
Monitor service quality and client health/safety; ensure plan authorization.
Consult on significant changes in physical health needs.
Coordinate resources during crises and residential/financial support planning.
Assist with Medicaid, SSI/SSD, food stamps and other benefits.
Provide rights information, guardianship alternatives, and proceedings.
Document accurately in clinical record per agency policy and Medicaid rules.
Ensure compliance with laws, rules, and procedures.
Investigate or report alleged abuse or neglect.
Collaborate with Recipient Rights Officer and MHHS Protective Services.
Transport clients to medical appointments when needed.
Meet continuing education and credentialing requirements.
Provide training programs for clients and community members.
Supervise support coordinator assistant when applicable.
Physical Requirements Walk, bend, stand, sit, lift up to 25 lbs; perform de-escalation training. Reasonable accommodations accommodated.
