Title: Physical Therapist (PT) - Home Health Services
Location: San Jose, CA
Duration: 26 weeks
Shift: Days, 5x8-Hour (08:00-17:00), Monday-Friday
Job Responsibilities
Provide physical therapy services to home health patients including geriatric and wound care cases Conduct patient evaluations, develop individualized treatment plans, and implement therapeutic interventions in patient homes Perform therapeutic exercises, functional mobility training, gait training, and balance / coordination exercises Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes Educate patients and caregivers on exercises, safety, and mobility strategies Document all patient care, visits, and progress notes accurately in Kinnser / WellSky while onsite Coordinate care with interdisciplinary teams including nurses and other therapy staff Travel within assigned territory to deliver care to patients' homes, ensuring timely and efficient visits Maintain professional standards and adhere to home health policies, procedures, and infection control practices Required Skills & Experience
Minimum 1 year of home health experience Minimum 2 years overall clinical experience as a licensed Physical Therapist Ability to work independently and manage patient caseload in a home health setting Strong communication and patient education skills Reliable transportation and ability to travel within assigned territory Punctual, self-motivated, and patient-care focused Required Certifications & Licensure
Current State Physical Therapy License (required, in hand) BLS - American Heart Association (required)
