We are currently seeking an
experienced Hospice RN Case Manager
to join our growing team in the Dallas market. This role is ideal for a compassionate, organized nurse who is passionate about delivering high-quality end-of-life care.
Responsibilities
Manage and coordinate care for a designated hospice caseload
Complete comprehensive patient assessments and develop individualized care plans.
Collaborate with physicians, social workers, chaplains, hospice CNAs, and other interdisciplinary team members.
Provide patient and family education, support, and symptom management.
Ensure accurate and timely documentation in accordance with hospice regulations.
Qualifications
Active Texas RN license
Hospice experience required (case management preferred)
Strong clinical assessment and time management skills.
Excellent communication and documentation abilities.
Valid driver's license and reliable transportation.
