Overview
Join BJC Home Care as a Hospice RN PRN. Provide skilled nursing, symptom management, pain control, emotional and spiritual support for adult and pediatric patients with life-limiting illnesses. Deliver care in patients’ homes or skilled nursing facilities across Missouri and Illinois.
Role Purpose Evaluate patient needs and implement individualized plans of care. Coordinate care across the continuum and serve as case manager when assigned.
Responsibilities
Assess patient preferences, values, emotional, spiritual, cultural needs.
Develop, implement, document individualized plans with defined goals in collaboration with interprofessional team.
Promote respect, equity, empathy in care delivery, practice collaborative problem solving, service recovery, advocacy.
Evaluate changes, inform and collaborate with family and interprofessional team, update EMR.
Maintain safety-sensitive alertness and safe work.
Minimum Requirements
Education: Nursing Diploma/Associate’s; RN license.
Experience: No experience needed; 2-5 years preferred.
Licenses & Certifications: Valid Driver’s License, RN.
Preferred Qualifications
Bachelor’s Degree, Nursing/Home Health.
2-5 years experience.
