Clinician – Home Based (Northeast Family Services)
Location: Lancaster, NH
Salary: $55,000 - $60,000 per year
Stay‑On Bonus: $1,500
Full‑time position available immediately throughout New Hampshire.
Responsibilities
Maintain assigned caseload and ensure proper implementation of in‑home services per DCYF referrals.
Collaborate with DCYF on referral requests, treatment planning, meetings, and clinical documentation.
Develop measurable treatment plans with families and DCYF.
Adhere to all documentation requirements, including assessment and report writing.
Report progress to the court system through appearances and written reports as requested.
Provide therapeutic intervention to children, adults, and families in home and community settings.
Deliver parent education and supervise visitation to children, adults, and families.
Act as clinical lead when multiple providers are assigned to a case.
Interface with DCYF, NH courts, schools, behavioral health providers, and other treatment team members as needed.
Participate in on‑call rotation, responding to phone calls and providing support to families as needed.
Be available in the early evening; most visits occur in afternoons or early evenings. Flexibility is required.
Assess client risk continuously and consult with the clinical supervisor on safety concerns.
Transport clients, including installing car seats and booster seats for children and adults.
Document all clinical contacts in the Electronic Health System.
Perform other duties as assigned.
Qualifications
Master’s Degree in social work, counseling, psychology, or a related field valid under NH state guidelines.
Minimum two years of clinical experience with children, adolescents, youth, and families in a home or community setting (preferred).
Valid driver’s license with reliable vehicle access and proof of insurance.
Bachelor’s Degree in social work, counseling, psychology, or a related field valid under NH state guidelines.
Minimum five years of clinical experience with children, adolescents, youth, and families in a home or community setting (preferred).
Valid driver’s license with reliable vehicle access and proof of insurance.
