OPCO Skilled Management is recruiting a PRN Hospice RN to join Del Corazon Hospice in Santa Fe, NM. We are dedicated to high‑quality, compassionate end‑of‑life care and seek a dedicated and compassionate RN to support our patients and families.
Job Summary As a PRN Hospice RN, you will provide high‑quality, patient‑centered care to individuals with serious illnesses. You will work collaboratively with our interdisciplinary team to manage pain and symptoms, and provide emotional and spiritual support to patients and their families. This is a per diem position, allowing for flexibility and autonomy in your scheduling.
Responsibilities
Assess patients’ physical, emotional, and spiritual needs.
Provide direct patient care, including wound management, medication administration, and symptom management.
Collaborate with patients, families, and interdisciplinary team members to ensure coordinated care.
Educate patients and families on disease processes, treatment options, and self‑care techniques.
Document patient care and communicate with team members, physicians, and other healthcare professionals.
See patients in varied settings: homes, assisted living, and long‑term care facilities.
Requirements
Graduate of an accredited school of nursing.
Current Registered Nurse licensure in the state of New Mexico.
One year RN experience in one of the following within the last two years: acute care, home health care, community or public health, or hospice.
Current BCLS certification.
Current TB testing, pre‑employment drug screening, and a clean criminal background check.
Valid state driver’s license and reliable automobile with proof of current automobile insurance.
Hospice Experience Preferred Experience in hospice care is preferred and will be considered an advantage.
