Per Diem Registered Nurse – Hospice Ocean County Job Locations: Ocean County

As a
Per Diem Nurse Hospice
you’ll assess patient needs, manage symptoms, create individualized care plans, and support families through the end‑of‑life journey.
Responsibilities
Complete a two‑week orientation focused on hospice care practices, EMR systems, and safety protocols.
Assess patients with advanced illnesses and create individualized hospice care plans.
Provide education, emotional support, and guidance to patients and families as they navigate the end‑of‑life journey.
Collaborate closely with VNA clinicians and external providers to ensure seamless, compassionate care.
Document patient status updates and adjust care plans as needs and goals evolve.
Qualifications
RN with Bachelor’s degree in Nursing.
Certification in Hospice Palliative Care Nursing and Critical Care skills preferred.
1+ year of supervised clinical experience as a registered nurse.
Excellent communication, strong computer, organizational, and interpersonal skills.
Working Conditions/Physical Demand Working indoors/outdoors with clinical equipment and use of phone and computer; car and valid NJ Driver’s license required.
Job Characteristics: Client/Customer contact.
Pay Transparency In compliance with New Jersey’s Pay Transparency Act, the total compensation range for this position is USD $60,000.00 to USD $110,000.00. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate’s work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add‑on incentives, special clinical compensation, retirement plans, and all other benefits.
