The Pharmacy Technician II, Medication Reconciliation provides support to pharmacy operations under the supervision of a licensed pharmacist.
Primary Duties and Responsibilities
Conduct in‑person or phone meetings with patients to update medication lists prior to their provider appointments, ensuring accuracy and completeness of the medication list.
Practice as a pharmacy technician in accordance with all federal, state, DFCI and Joint Commission policies, regulations and standards.
Create and update lists of current medications, document changes in dose, frequency, or route, and flag medications for removal to prevent harmful drug interactions.
Communicate drug‑drug interactions or other medication changes with pharmacists and providers to maintain patient safety.
Manage medication lists for medications listed from external entities, ensuring comprehensive and accurate records.
Utilize multiple systems such as EHR, Inventory Management Systems, and other systems to complete and document duties; troubleshoot as necessary.
Train other employees as assigned.
Communicate and escalates issues or problems.
May be asked to provide cross‑coverage in the hospital outpatient/specialty pharmacy as needed to maintain seamless patient care.
Perform other duties as assigned.

Minimum Job Qualifications Minimum Education
High School graduate or equivalent.
Minimum Experience
Two years of prior work as a pharmacy technician in a retail or hospital environment required.
Experience with oncology medications preferred.
License/Certification/Registration Required
Massachusetts Board of Pharmacy Registration as a Pharmacy Technician required.
National Pharmacy Technician Certification (PTCB) must be obtained within 1 year of hire date.
