We are seeking a qualified Nurse Practitioner to service patients in the Rock Hill area. As a Provider, you play a critical role in delivering exceptional patient care in patients' homes. This full‑time, salary‑based position offers the opportunity to receive a monthly bonus.

Responsibilities

Evaluate high‑risk patients with complex medical needs and develop individualized care plans.

Coordinate care activities with members of the interdisciplinary team to ensure continuity of care and optimal patient outcomes.

Communicate regularly with case managers, clinical coordinators, social workers, specialists, pharmacists, health coaches, and other team members.

Participate in interdisciplinary team meetings to discuss patient progress, treatment plans, and care coordination efforts.

Foster effective communication and collaboration to facilitate seamless patient management.

Meet staff efficiency expectations while providing high‑quality care to patients in various settings, including homes, facilities, telehealth, clinics, and specialties.

Document care management activities accurately and efficiently to support patient care plans and regulatory compliance.

Provide patients with education and resources related to their conditions, treatment plans, self‑management strategies, and preventive care measures.

Offer support and guidance to patients and their families to enhance their understanding of healthcare options and promote active participation in their care.

Adhere to all relevant healthcare regulations, policies, and procedures, including HIPAA and Medicare guidelines.

Monitor quality measures and gaps in care, identifying opportunities for improvement and implementing corrective actions as needed.

Qualifications

Nurse Practitioner (NP), Physician Assistant (PA), or Physician with a valid license to practice in the state of employment, license in good standing.

Demonstrated experience in providing patient care, preferably in a home health, clinic, facility, telehealth, or specialty care setting.

Strong communication, collaboration, and interpersonal skills, with the ability to work effectively within an interdisciplinary team.

Ability to evaluate high‑risk patients, develop care plans, and coordinate care activities to meet patient needs and improve outcomes.

Commitment to delivering compassionate, patient‑centered care and promoting a culture of excellence in healthcare delivery.

Strong organizational and time‑management skills, with the ability to multitask and prioritize responsibilities effectively.

Ability to read and communicate effectively.

Strong written and verbal skills.

Basic computer knowledge.

Leadership skills and ability to foster a cooperative work environment.

Team player able to manage multiple responsibilities and demonstrate sound judgment.

Must be able to work flexible hours, travel between offices and facilities, and be a licensed driver with an insured automobile in good working order.
