Position Summary The Wellness Nurse (LPN) provides high‑quality care through health assessments, medication management, and coordination with residents, families, and the care team. The role is supervisory, PRN, and primarily operates on the day shift. Weekend availability is a plus.

Key Responsibilities

Perform resident assessments and reassessments according to policies, monitoring any changes in condition.

Communicate with the Health and Wellness Director to ensure consistent quality of services.

Provide hands‑on care, medication administration, and specific nursing tasks when necessary.

Schedule and assign caregiving and med tech shifts to maintain adequate coverage.

Supervise, train, and support care team members, promoting teamwork and adherence to care standards.

Assist in transcribing physicians’ orders and reviewing new orders.

Maintain accurate and up‑to‑date resident records, including care plans, incident reports, and medical documentation.

Coordinate care planning with home health agencies on‑site.

Address residents’ special needs, requests, and complaints.

Coordinate admissions with the sales team, including supervising move‑ins.

Understand community care regulations and state‑specific regulations.

Facilitate communication between residents, families, and healthcare providers.

Candidate Qualifications

A valid, incumbent nursing license (LPN/LVN) is required.

High school diploma or equivalent.

Associate degree or higher in nursing is preferred; additional coursework in management is an asset.

Current First Aid and CPR license or ability to obtain.

At least four (4) years in geriatric nursing, including medication administration and pharmacy management.

At least one (1) year of supervisory experience, including staff scheduling and management.

Experience with memory care is a plus.

Outstanding organizational skills.

Positive attitude, empathy, patience, and commitment to treating residents with dignity and respect.

Ability and willingness to participate in on‑call rotation.

Must meet all mandatory health requirements according to state regulations.

Employment Type Part‑time – PRN, day shift with possible weekend availability.
