Hospice MSW PRN
Position Type: PRN (As Needed)
Schedule: Scheduled days per week – TBD.
Minimum Availability: Must be able to work a minimum of 10 hours every 90 days.
Visiting Nurses Association (VNA) is seeking a compassionate and experienced PRN Medical Social Worker (MSW) to provide psychosocial assessments, counseling, and resource coordination to hospice patients and their families. This role supports patients through end‑of‑life care by addressing emotional, social, and practical needs in accordance with physician orders, hospice plans of care, and regulatory requirements.
Key Responsibilities
Conduct comprehensive psychosocial assessments for hospice patients and families
Provide emotional support, counseling, and crisis intervention related to end‑of‑life care
Assist patients and families with advance care planning, community resources, and bereavement support
Collaborate with nurses, physicians, chaplains, and the interdisciplinary hospice team (IDT)
Participate in care planning meetings and case conferences as required
Ensure compliance with hospice, Medicare, state, and agency regulations
Complete documentation accurately and within required timeframes
Participate in required compliance training and departmental development activities
PRN Pay Guidelines
Hospice MSW Visit: $50.00 per visit
Qualifications
Master's degree in Social Work (MSW) from an accredited program
Current, unrestricted social work licensure in the State of Kansas (LSCSW, LMSW, or LCSW, as applicable)
Minimum of one (1) year of hospice, home health, or healthcare experience preferred
Knowledge of hospice philosophy, end‑of‑life care, and community resources
Strong communication, documentation, and organizational skills
Ability to work independently while collaborating with an interdisciplinary team
Valid driver's license, reliable transportation, and auto insurance
