What You Will Do
Under the supervision of the Program Coordinator, provide community‑based services to youth and adults with mental health and substance use disorders.
Deliver a scope of mental health and substance use related case management services, including ongoing assessment of needs, skill facilitation for personal independence and daily living skills, coordination of the Individual Treatment Plan (ITP), symptom monitoring, advocacy and outreach, intervention for symptom management, and activities to promote recovery and integration with self‑help programs.
Provide crisis support and stabilization activities—de‑escalating persons in crisis, assisting therapists with hospitalizations, actively assisting with safety planning, and coordinating with treatment team and after‑hours services as needed.
Offer Day Treatment services to adults as assigned by the Program Coordinator.
Work volume and target populations will vary by site/department needs and will be assigned by the Program Coordinator.
Render all services within departmental guidelines and in compliance with Medicaid/Medicare, HFAP, OhioMHAS, and CARF standards.
Engage in professional growth and agency‑sponsored learning opportunities, applying best practices appropriate to the conditions being treated.
Perform tasks that ensure patient safety, including screening and documentation of risk factors, developing safety plans, and coordinating with the treatment team.
Promote patient recovery through active collaboration in treatment planning and by involving natural support persons, self‑help resources, and community integration activities.
Provide assertive outreach when clients are not engaging in treatment services.
Comply with Patient Care Standards (PCS) and actively respond to supervisory strategies when challenges arise.
Support revenue generation through timely and accurate documentation and managing treatment plans; perform grants or special project tasks as assigned.
Utilize EMR and video‑conferencing software efficiently; maintain daily use of work email and stay current with FRMC communications. For new hires, these skills are expected by the end of the probationary period.
Communicate PHI‑related electronic forms in compliance with HIPAA and 42 CFR laws.
Contribute positively to patient, referral source, and employee satisfaction; demonstrate teamwork, flexibility, and leadership; participate in performance improvement and quality initiatives.
Effectively and efficiently provide care to behavioral health patients of all ages and cultural backgrounds.
What You Will Need
Bachelor’s degree in psychology, social work, or a related field (required); experience with mental health or substance use problems preferred.
Heart Saver, Aegis Crisis Prevention, and First Aid certification within 6 months of hire.
Strong written and oral communication skills; cultural sensitivity.
Punctual attendance at assigned work location.
Validity: Mobile with a valid Ohio motor vehicle license and insurance coverage; proof of personal automobile liability insurance upon request.
Successful completion of a 180‑day probationary period.
