Outpatient Pediatric Physical Therapist - Lake Zurich Base pay range: $40.30/hr - $60.45/hr

$2500 sign on bonus to qualified hire.

Responsibilities

Interpret physician referrals, complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods.

Establish a plan of care and implement individualized treatment based on evaluation results and functional goals, using appropriate techniques.

Monitor and evaluate outcomes, adjusting the individualized treatment plan appropriately, and provide status updates to physicians and other teammates.

Identify factors affecting patient’s physical and psychosocial comfort and attend to those needs.

Plan for patient discharge: provide written home programs, patient/family teaching, referral for community services, transition planning within continuum of care, and order equipment as indicated.

Communicate and collaborate with stakeholders to assist in achieving optimal care for the patient.

Complete all required documentation following department policy and accreditation standards with appropriate co‑signatures, including patient evaluations, progress notes, team conference reports, and discharge summaries as appropriate.

Record patient care and progress of each patient assigned; complete daily statistics, billing, and scheduling.

Apply insurance guidelines regarding authorization and number of visits; maintain site/department established productivity standards; understand and adhere to billing/documentation policies, procedures, and timelines.

Clean up after patient treatment, put away equipment, keep area neat. Perform manual techniques, patient positioning, transferring, and transporting duties requiring lifting and pushing/pulling, using proper technique.

Demonstrate skill in use of modalities, exercise equipment, and other devices, according to department orientation checklist. Demonstrate knowledge of indications and contraindications.

Promote patient safety by participating in safety initiatives and following all safety and infection control measures.

Participate in performance improvement activities to identify and resolve problems affecting patient care outcomes and improve quality.

Manage caseload; inform site leadership of coverage needs and ability to cover; assist with coverage based on department or regional needs.

Pursue professional growth through continuing education courses, departmental in‑services, and reading professional literature.

Orient new staff to clinical and department policies as requested. Participate in evaluation of co‑workers by providing verbal/written feedback.

Supervise physical therapy assistants, extenders, and students as applicable, delegating patients, providing direction, recommendations, and ongoing supervision while maintaining responsibility for patient plans.

Minimum Job Requirements Education Doctorate degree in Physical Therapy from an accredited school; ongoing continuing education required.

Certification / Registration / License Licensed Physical Therapist in the State of employment; Basic Life Support (BLS) for Healthcare Providers obtained within 90 days unless otherwise determined.

Successfully complete all competency assessments/requirements for patient care and site standards

Maintain annual educational requirements for hospital‑based certifications (e.g., DNV, CARF)

Knowledge of principles of growth and development over lifespan and age‑specific assessment skills

Understand pathophysiology and principles for pediatric care

Physical Requirements And Working Conditions

Must be able to sit, stand, walk, lift, and squat throughout the workday

Lift up to 50 lbs. from floor to waist; up to 10 lbs. from waist to overhead; up to 40 lbs. at waist height over a reasonable distance

Push/pull with 30 lbs. force; perform sliding transfer of 150 lbs. with a second person present

Repetitive hand use: simple grasping (5–15 lbs); pushing/pulling up to 75–100 lbs.

Fine manipulation during massage and joint mobilization

Functional use of senses for effective communication

Exposure to electrical and chemical hazards, blood and body fluids; must wear protective clothing as needed

Condition may require travel; potential exposure to road and weather hazards

Operate all equipment necessary for job duties
