Description
This is a per diem/on call inpatient Social Worker position supporting patients at Santa Rosa Memorial Hospital, with potential assignments at Healdsburg and Petaluma Valley.
The role provides psychosocial assessment, short‑term counseling, emotional support, goals‑of‑care conversations, and advance care planning for patients with serious or life‑limiting illness and their families. You will work within an inter‑professional team to identify problems and help patients/families explore realistic options, make decisions, and solve problems.
Required Qualifications
Master’s Degree in Social Work.
1 year of experience in healthcare (hospice, palliative care, or acute care preferred).
Preferred Qualifications
Social Worker License upon hire.
