Job Duties

Plans, organizes and supervises the social services program of the Arizona State Veteran Home.

Ensures timely completion of assessments and documentation per federal and state regulations and facility policies and procedures.

Ensures compliance with the PASRR (Preadmission Screening and Resident Review) process.

Supervises subordinate staff to ensure the goals and objectives of the work unit are met.

Plans, organizes, assigns, and reviews the work of the Social Services staff.

Reviews and evaluates job performance of staff through monthly 1 on 1 coaching sessions and annual performance appraisals.

Addresses staff problems and recommends disciplinary actions.

Interviews and selects staff.

Oversees clinically appropriate discharge planning to ensure it is safe and communicated effectively to all involved stakeholders.

Monitors Medicare notices to ensure timely delivery to beneficiaries per federal law.

Completes consults for referrals to specialist at the Veterans Administration (VA).

Works with residents, families, and staff to ensure that veterans/members are treated in accordance with Resident’s Rights.

Assists in projecting department budget working within budget parameters and monitors expenditures.

Maintains a personal caseload of assigned veterans/members.

Attends and participates in various meetings and serves on committees.

Knowledge Of

Veterans Administration, CMS, and State regulatory standards.

Social Services programs and principles of social work.

Geriatric psychiatric issues.

Geriatric disease processes.

Side effects of psychotropic medications.

Physical and chemical restraints.

Community resources.

Governmental policies and programs that affect our clientele.

Long-term care billing standards.

MDS system.

Skills In

Ability To

Establish and maintain interpersonal relationships at all organizational levels.

Utilize the facility's IT program and maintain and record assessments and care plans.

Maintain the highest level of confidentiality.

Selective Preference(s)

Master’s degree in social work.

One year of supervisory experience in a health care setting.

Experience in long-term care.

Pre-Employment Requirements

Active social work license issued by the Arizona Board of Behavioral Health Examiners upon placement into position.

Ability to obtain and retain a valid AZ DPS Fingerprint Clearance Card.

Ability to drive a vehicle in the course of conducting State business and possession of a current, valid, state-issued driver's license.

Current Negative Tb skin test (Mantoux skin test) within the last 6 months or a written statement from a physician, physician’s assistant or a registered nurse practitioner indicating freedom from tuberculosis, if in the past has had a positive skin test or tuberculosis.
